We know that you have a broad choice of providers for your health care needs and appreciate that you have trusted Performing Arts Physical Therapy with your wellbeing and career.

Please take a moment to complete the appropriate documents in the tabs to the right. Completed forms may be emailed to info@performingartspt.com.

You should count on your intial evaluation to last at least one hour. Subsequent visits are 30 minutes each. The frequency and duration of your physical therapy treatment is determined by your MD, your insurance provider, and/or your treating therapist.

To expedite the process of your first visit, please take the time to view the appropriate tab, download the checklist and intake forms, and bring the completed forms with you to your appointment. Please also bring your Insurance card, prescription, referral, or C2 form (worker's comp) as appropriate.

You should also bring appropriate clothing to your initial visit and follow up visits that allow the therapist sufficient access to the affected body part (i.e., shorts for lower extremity injuries; a tank top for back, neck and upper extremity injuries).

Late Cancellation/No-Show Policy
Performing Arts Physical Therapy has a 24-hour notice requirement in the event of cancellation of any appointments. If an appointment is not canceled 24 hours prior to the time of the appointment, or the appointment is missed for any reason, the patient will be charged a fee of $50.00 for each missed appointment. Patients who frequently Late Cancel or No Show may forfeit their privilage of scheduling in advance.


Insurance Policy Statement
The Patient understands that he/she is financially responsible for all Physical Therapy services not covered by the Insurance Company.

We accept the following insurances: In Network: AFTRA Health Fund, Cigna PPO, Empire BC/BS, Gallagher Bassett Service, Oxford, SAG, Cigna Open Access. Out of Network: Aetna, HealthNet/OrthoNet, Health Plus, United Healthcare. We accept the following Worker's Compensation insurances: AIG, AIU, Chubb, Fireman's Fund, Liberty Mutual, NY State Insurance, State Insurance Fund, Risk Management, Travelers, The Hartford Ins, and Electric Insurance Company.
