New Patient Info

Mr. Gallagher is currently only seeing patients one day a week. He is also only providing care to those without insurance.  

Please take a moment to complete the appropriate documents in the tabs to the right. Completed forms may be emailed to spgpt@performingartspt.com.

You should count on your intial evaluation to last at least one hour. Subsequent visits are 30 minutes each. The frequency and duration of your physical therapy treatment is determined by your MD, and/or your treating therapist and your specific injury needs.

You do not need a prerscription to receive physical therapy. New York state law allows you to receive physical therapy services for 10 visits or within a 30 day time frame which ever come first.

To ensure the process of your first visit, please take the time to view the appropriate tab, download the checklist and intake forms, and bring the completed forms with you to your appointment. If you do not have the forms filled out when you arrive you will not be seen. Please also bring your prescription if you have one

You should also bring appropriate clothing to your initial visit and follow up visits that allow the therapist sufficient access to the affected body part (i.e., shorts for lower extremity injuries; a tank top for back, neck and upper extremity injuries).

Late Cancellation/No-Show Policy
Performing Arts Physical Therapy has a 24-hour notice requirement in the event of cancellation of any appointments. If an appointment is not canceled 24 hours prior to the time of the appointment, or the appointment is missed for any reason, the patient will be charged The fullfor fee each missed appointment. Patients who frequently Late Cancel or No Show may forfeit their privilage of scheduling in advance.


Insurance Policy Statement
The Patient understands that he/she is financially responsible for all Physical Therapy services as PAPT and Mr. Gallagher does not accept insurance for payment. This is a Cash/Check based practice.
